jgrusewski 1f1412e08d feat(wave-d): Complete Wave D Phase 6 with 240+ parallel agents
Wave D regime detection finalized with comprehensive agent deployment.

Agent Summary (240+ total):
- 153 core agents: D1-D40, E1-E20, F1-F24, G1-G24, 45 cleanup
- 87 extra agents: T1-T3, S2-S8, R1-R3, M1-M2, D1, E1, P1, TLI1, DOC1, Q1, CLEAN1
